Sampling decision

What a sampler returns for a new span: drop it, record it only, or record and sample it.

Only RECORD_AND_SAMPLE sets the sampled flag, so the span is exported and the flag travels in traceparent. RECORD_ONLY keeps the span for local processors without exporting it.
